Your Healthcare practitioner uses the Amplitude Clinical system to securely collect and store your personal data, health information and clinical notes electronically. Through the Amplitude Clinical Patient Portal you will be asked to complete your initial Intake Form online. This will allow your practitioner to look through your medical history and other information before they see you, helping to save time on the day of your appointment!

The Patient Portal also allows your practitioner to monitor your progress throughout your course of treatments with occasional health questionnaires sent via email. This helps ensure that your practitioner is fully addressing your main health complaints, and that you are being provided with the best quality care possible. We are required to ask for your consent to collect, store and process your personal information and health data electronically in this manner, in accordance with GDPR, HIPAA and other relevant international guidelines.

This is all done securely and confidentially with the Amplitude Clinical system, where we adhere to the highest international standards for protecting and processing patient health data. The Patient Information Sheet explains clearly how your data will be stored, processed and protected.
